- Are new patients accepted?
Yes! We welcome new patients. You are advised to read and complete the following forms and provide them during your visit: Patient Registration form, Consent for Medical Services, Consent for message Authorization, Delta Pediatrics Notice of Privacy Practices form, Vaccine Administration Consent form, and the No-Show Policy. - Do you accept walk-ins?
No, we prefer you call to see if there are available appointments for that day. - Which hospitals are you affiliated with?
Dr. Mordi is affiliated with Gwinnett Medical Center and Children’s Health Care of Atlanta Scottish Rite/Egleston. - If I choose to use Delta Pediatrics as my baby’s pediatrician prior to delivery, what do I need to do?
You will inform Gwinnett Medical Center that you have chosen Delta Pediatrics as your baby’s pediatrician. - What is your area of specialty?
Pediatrics - Is the physician board certified?
Yes, Dr. Mordi is a board certified physician. - Do you accept cash payments?
Yes. Cash payments are accepted. If you are a self-pay patient, we may negotiate a discounted fee; this will be discussed with you at the time services are rendered. - What are your office hours?
Monday-Friday 9:00am-5:00pm with lunch time hours of 12pm – 1pm, closed on Saturdays and Sundays. - Do you have sick and well waiting areas?
Yes! We have separate well and sick waiting areas as well as a separate room just for newborn babies and breastfeeding moms. - How can I get my prescriptions refilled?
Depending on the Rx, you can have the pharmacy fax over refill request. You can also call us at 678-765-6749 to request for a refill or email us at info@deltapediatrics.com. - Can I contact you after office hours?
Yes. There is an answering service that would contact the on-call provider. - What Lab tests do you offer?
We perform most of our lab tests in-house. - What insurance companies and plans do you offer?
BCBS, UHC, MEDICAID, AETNA, TRICARE, CIGNA, AMERIGROUP etc. For a complete listing of insurance plans accepted, please check out the “Insurance Plans” tab on our website. - Do you assist with referrals?
Yes! We do all referrals for the patient. - Do you have a parking area?
Yes, we have free parking adjacent to building. - Can my child be seen without a parent/guardian being present?
If the child is sick and is accompanied by another adult with a written permission from either parent/guardian, the child will be seen. For a well checkup, a parent/guardian must be present for the child to be seen. Only a child for emergency treatment can be seen without a parent or guardian being present. - Can I request for an appointment online?
Only non-emergency appointments should be requested online. If your condition is critical, please call 911. - I received a bill from your office what do I do?
You will only receive a bill if we received a statement from your insurance company stating that you have an outstanding balance due. Any outstanding balance after a statement is received from your insurance is your responsibility. - What are forms 3231 and 3300?
Form 3231 is the Georgia Immunization Certificate and 3300 is the Hearing, Vision and Dental Form.
